The mat b1 form is one of those things you don t know you need until you suddenly need it.

If your employer starts asking for it before then it might be worth leaving a message at your gp for the midwife to pick up when they re there next with your name due date and contact number they might be happy to leave the completed form for you at the surgery reception or can.
For this reason we recommend making a copy as soon as you sign it and keeping it in a safe place before the baby brain sets in.
